This probably isn't entirely right, but here's what i think this means: native severe triple vessel disease is the original heart problem prior to the previous bypass surgery - if this is correct, this observation is a summary of his history and not related much to any present problems, but his current problems look like they are pretty much the same thing as the original problem.

The svg graft consists of the veins that were grafted to his heart at the previous surgery. They are occluded, which means they are not supplying enough blood to the heart muscle.

LAD , LIMA, LAX, RCA, are abbreviations for the arteries that supply blood to the heart. The first three have blockage or narrowing towards their ends (which is better than blockage at the beginning), and the right coronary artery has blockage or narrowing near the beginning.

The advice for clinical management means that the docs will need to perform some type of procedure to open the arteries, replace them again, or use some other alternative.

Overall, it looks like your father's heart has blocked arteries to parts of the heart but the good news is that he has "good lv function" which means that the heart is still pumping well, so the blocked arteries haven't caused enough damage at this point to affect the heart function. That means that he could have a very strong heart if the coronary artery problem is fixed.
