It is difficult for VERY pregnant women to properly cleanse themselves with a towelette, much less be able to collect a clean catch specimen. There is a great deal of normal bacteria in that area, so if the woman did not cleanse or do a clean-catch, then of course, the urine culture is going to be loaded with bacteria.
also, sometimes a person is just lazy, doesn't follow instructions, etc.
To determine if someone has a UTI, other results of the urinalysis need to be observed...was there a large number of white blood cells.? Was the urine specimen nitrite positive.?
A good thing to look for to know if a specimen is a clean catch is the epithelial cells present. If there is a large number of squamous, that indicates vaginal contamination. (Transitional and renal obviously imply renal shedding).
