If you never had sex the chances of it being a sexually transmitted disease is highly unlikely. However if you have oral herpes and then masturbated it could have spread to your genital. Because the pimple like thing is probably a sore and yes your genitals hurt during a genital herpe outbreak. I would get it checked out if i were you.
