this is because the pancreas is in the retroperitoneal space - which in fact is part of the back.
a perforated stomach ulcer could produce similar symptoms - but you'd expect the patient to localize the pain to the upper abdomen, not the back.
without the fever, you might suspect an aortic dissection.
The kidneys are also in the retroperitoneal space, but as a rule, pyelonephritis is never described as back pain - it is described as flank pain.
vertebral osteomyelitis with paraspinal abscess would produce fever and back pain, but never nausea.
