There are lot of nerve endings in the area.. also the testes have little support but are considered, through evolution, to be important.. Without support they move around easy so when you hit someone in the balls there is a lot of movement, unlike when you hit someone in the arm where there is muscle and bone to support it..

It is VERY painful! The pain is natures way or telling us that this is an area we need to keep protected! With out properly working testicles there is no chance for having children! Whats worse is that nature seemed fit to put such an important part of a males body on the out side, this helps the testicles regulate temperature for the production of sperm..
