Ouch! Ok it can be anything and he should be checked my a physician.. The obvious thing that comes to my mind is Hemrrhoids.. They can be very painful and the pain can last for hours.. I am however concerned that it only happens when he has a bowel movement.. This can be a sign of a more serious problem.. Please have him see a doctor as soon as possible..Health Question & Answer
If he is suffering from constipation he should take more ruffage and carbohyddrates and less proteins and drink lot of water.. In the morning also take two to three glasses water after leaving the bed.. Instead of tea or coffee he should take mild hot water with lemon and honey.. These will remove his problems.. If some thing else you can ask him or take him to doctor..Health Question & Answer
The adult word is defecating..
It would seen that his diet is lacking in fiber.. I reccommend a large bowl of oatmeal every morning, plus additional fruits and vegetables in his diet.. I had a similar problem, with eating too many processed foods..
The higher fiber diet will soften the stool, and make defecating much easier..Health Question & Answer

High fiber diet is the answer.. also tell him to try not to strain.. The hot bath does help -- but 5 minutes in.. He can go back after a while for another.. It could be hemmeroids, it could be a fissure.. He may feel like he needs to "go" more..Health Question & Answer
