Though I'm not a male I know for sure that blue balls is a real problem for men. It's not necessarily if you don't get "any" but it's more of when the male is close to or beginning to orgasm and all stimulation to his penis or body stops, causing the semen to retrograde back into his testicles, causing the testes to ache and actually turn a slight bluish color. The testicles become full when a man hasn't had any sexual activity when he's not used to it. This can become painful also and the male will need to relieve himself also to alleviate this discomfort.
