Answers:
9 times out of 10 this problem can be self-treated privately at home. Rarely will a circumcision be necessary.
He needs to do some stretching exercises, ideally in the bath/shower, every day for a few weeks. Follow the instructions on this website:
http://www.geocities.com/restoremaine/ti...

He should be patient as it may take up to a month to see any improvement, and be disciplined to do the stretching regularly! If he struggles to make any improvement then he needs to ask his doctor for a topical steroid cream to put on the tight foreskin.

This website also has some more general advice:

http://circresearch.googlepages.com/tigh... provides stretching instructions to fix a tight foreskin.

However if he has a particularly tight band "phimotic ring" he might like to go to the doctor and ask if it can be cut surgically to loosen things up.
