Nope, no MRI or anything fancy.
When I was diagnosed as a teenager, my parents had their concerns, and asked my doctor at a sports physical. He asked me a series of questions about some habits I had, and they pointed to me having OCD. So all they do is ask you questions and determine it themselves, or they refer you to someone else like a therapist.

There are pills, but they are often those that are used for depressions, so they don't treat OCD as well or directly as they should. And many people find the don't help. I was one of them.
