Lactose, a sugar, and milk fat are two different things. You can lose your ability to digest either as you get into adulthood.
It is easy to test. Go without milk for a week. When you have plenty of time to endure symptoms, drink a big glass of whole milk. Skip a week again. Try it again with lowfat milk.
I myself can drink all the 2% milk and cheese I want, but get symptoms from whole milk or ice cream. I was mis-diagnosed as IBC for about a year when it started, but my new dorm-mate told me about milk fat intolerance and it all cleared up. My brother-in-law can eat NO milk products, fat or not. He is intolerant to lactose.
The recommendations are to eat small amounts that you can tolerate, frequently throughout the day, for both lactose and milk-fat intolerance.

The "whole" or "Fat" or "skim" in milk refers to the amount of milk solids in the product (hence skim milk is much more watery) ONLY a doctor or equiv. medical prof. can say, but it does sound like an intolerance to those milk solids, and skim milk is easier to drink because there aren't as many.
